Short explanation:
When someone creates a report in a project and gives the report status "CLOSED", the system will distribute an e-mail alert to the recipients who are added in the alert list of that project for that specific form template.
What you need:

An active Forms Pro Module in CHECKD

Be an administrator or owner of a project
Steps:

Navigate to your desired project

Go to Project Info

Scroll down to FORMS notification/alerts rules

Select person who should receive mail alert

select type of Form Template that they will receive a mail for

Press save
How to use:

Open report (Works on Field and WEB)

Fill out report
-
Press save
-
Press the 3-dotted menu on top right

Select CLOSE Report

Now the system will distribute an e-mail with report information.
What happens when you close a report?

The report is locked and can't be changed.
