This article explains how to enable anonymous reporting for forms (checklists), ensuring that completed submissions do not include any identifying information about the respondent. In some situations, you may want a form to be filled out anonymously — meaning it won't include any information about who completed it.
If you'd like to make a form anonymous, just follow these steps:

Create a new form, or open an existing form that is currently in Draft mode (not yet published).

Inside the form, look at the preview section on the right-hand side and select the Template Options tab.

Under Template Options, check the box next to Enable anonymous reports.

When you're done, make sure to save the template.
